Parents, please register your children for Sunday School using one of the forms below.
You may register all the children in your family on one form.

Please complete this online form and save it. 
  • If you have Adobe Acrobat installed, you will be able to edit the signature line. Please sign the form using Adobe Acrobat and save it.  (Rename the file to include your Last Name in the file name, please).
  • If you don't have access to Adobe Acrobat: please save the unsigned document. (Rename the file to include your Last Name in the file name, please). The Parish Office will print it and look for you during Coffee Hour so you can sign it.
